Listed below are a series of tools and resources developed by CDC-funded contractors and grantees that previously were, or currently are, funded to provide Data to Care technical assistance to state and local health departments. The tools and resources presented in this section were created for specific technical assistance requests and are being presented in their original format without further editing or review by CDC. Jurisdictions should carefully review these materials and determine their feasibility and applicability for their Data to Care programs. Resources can be modified to meet jurisdictions' needs.
