Data to Care Assessment Tool for Health Departments

The Data to Care Assessment Tool for Health Departments is designed to assist health departments and their partners throughout all phases of developing and implementing a program that uses HIV surveillance data to conduct linkage to and re-engagement in care activities. The tool is a guide to assist health departments in thinking through and conducting important developmental steps; however, the tool is not exhaustive and health departments likely will identify other important developmental steps as they develop a Data to Care program.

Prior to using the tool, health departments should review and familiarize themselves with the Data to Care material on effectiveinterventions.org that describes important program development and operational considerations, data sources and quality issues, and health department Data to Care program examples. Health departments will find the tool more intuitive once staff have a basic conceptual understanding of a Data to Care program.

The materials on this site are designed for HIV/AIDS prevention with persons at risk for acquiring or transmitting HIV. They are meant to be resources used by HIV prevention providers such as health departments and community-based organizations so as to provide the best evidence-based HIV prevention services. These materials are not meant for the general public. They are not meant for children. They are not school-based HIV prevention strategies.
